Please provide us with a 24 hour notice of cancellation so that we may utilize our schedule to provide better patient care. If you don’t offer at least 24 hours advance notice, we may charge you a $50.00 missed appointment fee. This charge will not be billed to your insurance company. Please be advised that if you miss two appointments without providing notice, you will be required to pay a $100 scheduling fee for future appointments. This fee will be applied toward the cost of your appointment. If you miss your appointment and do not provide required notice, this scheduling fee will be forfeited.
